Groups frequently check or sign attendance sheets as required by court order or from a therapy center as a courtesy to members. However, some groups and members choose not to do so. If necessary, it is best to ask how the group handles this before the meeting begins.

Please complete the updated ‘Staff Security Screening, Consent and Authorization Form’.
In Part B, you will see that you must enter the addresses of your last five years. If you have lived at more than two addresses during these years, an Appendix sheet is attached to the document at the end for you to enter them.
In Part C, it is very important to write your handwritten initials in the ‘Person’s Initials’ box on lines 1, 2 and 3. And your signature and the date.
On February 1, 2015, the Correctional Service of Canada (CSC) implemented a mandatory credit check for all new security screening applications. When completing the Personnel Security Screening, Consent and Authorization Form (TBS 330-23), please ensure that the applicant initials box 3 in Part C to consent to a credit check. If the applicant refuses to consent and initial boxes 1, 2, and 3, the security screening will be terminated and the process cancelled for administrative reasons.
